Testosterone decanoate and testosterone undecanoate are both esterified forms of testosterone, mainly used in testosterone replacement therapy (TRT) to treat low testosterone in men. The differences between these two drugs are mainly reflected in the following aspects:
### 1. **Chemical structure**
- **Testosterone decanoate**: a fatty acid chain with 10 carbons in the molecule.
- **Testosterone undecanoate**: a fatty acid chain with 11 carbons in the molecule.
This difference in ester chain length directly affects the release rate and metabolic characteristics of the drug.
### 2. **Release and duration of action**
- **Testosterone decanoate**:
- Faster release time, but still a long-acting ester.
- Often used in combination with other esterified testosterone (such as propionate or isocaproate), for example in compound preparations (such as Sustanon).
- Half-life is usually 7-10 days, requiring more frequent injections (usually once every 2-4 weeks).
- **Testosterone Undecanoate**:
- It is one of the slowest-releasing testosterone esters known to date.
- With a half-life of 18-21 days, the injection frequency is usually once every 10-14 weeks, which is very suitable for patients who need long-term stable testosterone levels.
### 3. **Usage**
- **Testosterone Decanoate**:
- Mainly injected intramuscularly.
- Commonly used in combination preparations, not alone.
- **Testosterone Undecanoate**:
- Can be injected intramuscularly or orally (such as Andriol). Oral preparations are absorbed into the lymphatic system through chylomicrons, avoiding the first-pass effect of the liver, and are more convenient to use.
- Long-acting intramuscular injection preparations are more widely used in the treatment of chronic low testosterone.
### 4. **Therapeutic Uses and Indications**
- Both are used for testosterone replacement therapy, but testosterone undecanoate is more suitable for patients who need fewer injections and long-term stable efficacy due to its ultra-long-acting properties.
- Testosterone decanoate is more often used as part of a combination preparation to provide a medium-effective blood drug concentration maintenance.
### 5. **Side effects**
- The side effects of both are basically similar, including possible water retention, acne, polycythemia, etc., which are specifically related to the patient's dose and personal tolerance.
- Due to the more stable release curve of undecanoate, adverse reactions caused by fluctuations in blood drug concentration are usually better avoided.
